Selective Tibial Neurotomy in Patient with Post-traumatic Spastic Equinovarus

Abstract
Although spasticity is one of the most frequent sequelae encountered in neuro-rehabilitaional fields after brain damages from various etiology, the neurosurgical treatments for spasticity have been infrequently undertaken in Korea. Authors report selective tibial neurotomy in 44-year old man with spastic equinovarus developed after head trauma and consequent gait disturbance. Preoperatively the patient showed generalized spastic changes in all extremities, but his main disability in daily living was gait disturbance due to right spastic foot. His right tibial nerve was operated selectively at popliteal fossa under the surgical microscope, and remarkable improvement in gait pattern ensued due to postoperative complete heel contact. Selective tibial neurotomy is considered as a useful surgical technique in patients with spasticity from traumatic brain damages if chief disability results mainly from localized spastic problem.
